Position Summary

The Telehealth Nurse Practitioner (NP) delivers high-quality, patient-centered virtual care to members across diverse populations. The NP is responsible for diagnosing and managing acute and chronic conditions, prescribing medications, promoting preventive health, and coordinating care with specialists and community resources. A key component of this role includes ensuring accurate and complete clinical documentation to support risk adjustment coding and HEDIS quality gap closure, advancing both patient outcomes and organizational performance.

Key Responsibilities:

Clinical Care & Patient Management

  • Conduct virtual consultations to diagnose and treat episodic and chronic conditions.
  • Develop and implement individualized patient-centered care plans.
  • Prescribe medications and manage treatment plans within scope of practice.
  • Coordinate referrals to specialists, urgent care, complex case management, behavioral health, community resources, or in-person providers.
  • Educate patients and families on chronic care management, preventive screenings, and lifestyle modification.

Collaboration & Care Coordination

  • Collaborate with physicians, behavioral health providers, pharmacists, case managers, and other healthcare professionals to ensure continuity of care.
  • Participate in interdisciplinary team meetings to review patient cases and align care strategies.
  • Support integration of telehealth services into population health and quality improvement programs.

Documentation & Compliance

  • Maintain accurate, timely, and complete documentation of all patient encounters in accordance with organizational, NCQA, CMS, and state standards.
  • Apply appropriate ICD-10 coding to ensure accurate risk adjustment capture and support revenue integrity.
  • Identify and document preventive screenings, immunizations, and chronic condition management to facilitate HEDIS gap closure and quality reporting.
  • Adhere to telehealth best practices, HIPAA privacy/security requirements, and all state/federal regulations.

Qualifications:

Required

  • Master’s degree in nursing (MSN) or Doctor of Nursing Practice (DNP).
  • Active, unrestricted Nurse Practitioner license in Ohio and Arizona. Additional states may be required depending on business needs. 
  • National certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology NP.
  • Minimum 2 years of clinical experience as an NP in primary care, urgent care, or telehealth.
  • Demonstrated knowledge of risk adjustment coding (HCC/RAF) and HEDIS measures.
  • Proficiency in electronic health records (EHR) and telehealth platforms.
  • Reliable internet connection and private workspace.

Preferred

  • Experience in managed care, population health, or integrated delivery systems.
  • Familiarity with NCQA standards and quality improvement programs.
  • Bilingual or multilingual skills.

The ideal candidate is a clinically experienced NP with a strong foundation in both virtual care delivery and managed care documentation requirements. They are skilled at identifying and coding chronic conditions for risk adjustment, closing HEDIS gaps in care, and engaging patients in preventive health. They demonstrate adaptability, cultural competence, and the ability to collaborate across disciplines to improve member outcomes while supporting organizational quality and financial performance.
