Next Steps with your Quest Diagnostics Referral
As an Antidote Health insurance member, if your provider refers you to Quest Diagnostics, an in-network lab, follow these steps to complete your testing process smoothly.
Schedule Your Test
You can schedule an appointment at a Quest Diagnostics location through their website or by phone. Walk-ins are also accepted, but please note that appointments take priority over walk-ins.
Confirm Costs
As an Antidote Health member, Quest Diagnostics is an in-network provider, which means lower costs for you. You can check the estimated cost of your tests on the Quest Diagnostics website or by phone. If you have questions about coverage or out-of-pocket expenses, contact Antidote Health Member Services for assistance.
Antidote Elite members get $0 copay for routine labs at Quest Diagnostics when referred by an Antidote Virtual Provider.What to Bring
Photo identification: Bring a valid ID (e.g., driver’s license, passport, or other government-issued photo ID) that shows your first name, last name, and date of birth.
Insurance Information: Your Antidote Health insurance card is required to confirm coverage. You can find your digital ID card available in the Antidote App or Antidote Member Portal.
Payment Method: Have a credit, debit, or health savings card available to preauthorize payment for any balance due after insurance is billed.
After Your Test
After completing your test, Quest Diagnostics will deliver your results within 2–5 business days, depending on the test type. Once your results are available, we recommend scheduling an appointment with an Antidote Virtual Provider to review them and discuss next steps.

If you prefer a generic lab referral that can be used at any in-network lab facility, please select 'Get Non-Quest referral' next to your Quest referral in the Antidote App or Antidote Member Portal. We recommend using our Provider Search tool to find in-network providers.
