About the Program

High blood pressure, also called hypertension, doesn’t always have symptoms, but it can hurt your health if not managed. Our program is here to help you take control so high blood pressure doesn’t control your life. Your virtual provider will complete a blood pressure control assessment and work with you on a plan to lower it. They’ll help you figure out the best steps to get it under control and make changes if needed.
