What Are Metal Levels?
Bronze Plans
A Bronze plan pays for about 60% of your care. The monthly cost is low, but you pay more when you see a doctor. This is a good choice if you don't expect to need much medical care.
Silver Plans
A Silver plan pays for about 70% of your care. The monthly premium cost is in the middle. This is a good choice if you qualify for 'extra savings' (also called Cost-Sharing Reductions), which can lower your deductible, copayments, and coinsurance.
Gold Plans
A Gold plan pays for about 80% of your care. The monthly cost is higher, but you pay much less when you get care. This could be a good choice if you use medical services more often.

Who qualifies for a Catastrophic plan?
Historically, anyone under the age of 30 could purchase a Catastrophic plan without any additional documentation.
Good news for 2026. Eligibility has been expanded. Now, individuals over 30 who are not eligible for financial assistance (like premium tax credits) because their income is below 100% or above 400% of the Federal Poverty Level may qualify for a hardship exemption to enroll in a Catastrophic plan. This change provides a more affordable coverage option for those who might otherwise find health insurance to be out of reach.

If you are traveling outside your plan's service area and have not previously established care, please contact Member Services to confirm virtual care availability. Prescriptions, visits, and services may be limited at the clinician's discretion in accordance with state law or under plan terms.
*** $0 copay applies to designated Tier-1 primary care providers, up to 4 visits/plan year. Refer to the Antidote Provider Directory to confirm Tier-1 designation.
**** $0 copay applies to all medications on the $0 Core Rx Drug List when prescribed by an Antidote Health clinician, and to all medications on the $0 Advanced Rx Drug List regardless of the prescribing clinician. Refer to the applicable drug list for the most current information and pricing.
^ $0 copay applies to Tier-1 primary care provider (PCP) visits, up to 4 visits/plan year. When referred by an Antidote Health clinician, PCP visits are $0 regardless of Tier designation. Without a referral, the standard PCP copay applies. Referrals maybe limited when the requested service can be provided directly by an Antidote clinician or when a referral is not considered medically necessary. Members may choose to self-refer; however, doing so will result in the Tier-1 or Tier-2 copay applying. Specialist referrals are discounted based on Tier designation .See plan details for more information.
